First of all, thanks everyone for the input. The text is a short one-page document entitled "Departamento de Contabilidad, Autorización de Ingreso". All items are placed in vertical form to be filled out on the form (employee name, position, start date, department...) and the last thing, just before the signature, still as a vertical list, is what I posted in the query. Not sure what further context I can give. Maybe it's like Emma indicates, an additional payment, although there's already the "gratificación" which would seem to be that.
