"Memorial prize" is by far the most common and is generally a prize established in memory of a particular (deceased) person. "Commemoration prize" is more likely to be in memory of an event, but can also be in memory of a person. "Commemorative prize" is often used for a prize that is simply a tribute to the "winner" having participated, or that commemorates the event/competition itself.

Commemoration prize would be the prize given at a memorial event. A competitor could win the commemoration prize e.g. the Ashes, a prize which commemorates "the death of English cricket", the ashes being a memorial to a bygone cricketing nation, and receive a commemorative competitors medal since they have to give the memorial prize back the next time the contest is run.
