Basically, this means inefficiency, but I'm not sure this is the right word in the conext of an ISO audit. And I hate the way "issues" has become a near-universal euphemism for "problems", but there we go...

Here is an example supporting my suggestion:
"ISO/IEC 20000 forms a foundation on which enterprises can build a common vocabulary and integrated processes. Integrated processes are particularly important in a multi-sourced environment, where different vocabularies and processes can result in misunderstandings, poor hand-overs and service-quality issues."

I used to be a Quality Manager in an ISO certified company, and the nearest I can think of for what you might have here is the company's response to the auditor's findings, viz. non-conformances/non-conformities and 'observations', which are typically failures to follow the quality system or defects in the system itself. These do not have to be formal customer complaints, although the auditor will obviously focus on those.
