Dutch term or phrase: verklaring omtrent het gedrag

J'aimerais trouver une traduction satisfaisante de l'expression néerlandaise "verklaring omtrent het gedrag".
La traduction "certificat de bonne vie et moeurs" me semble un peu datée pour les lecteurs français et a en France un autre sens que celui de VOG.
"Extrait de casier judiciaire" ne correspond pas non plus tout à fait au contenu d'une telle déclaration.
Je pensais à "certificat de bonne conduite" mais cela ne me paraît pas non plus la meilleure solution.

C'est bien comme ça qu'on l'appelle, mais les mairies ne sont plus obligées de le délivrer (voir le lien de la ville de Paris)

Beaucoup de choses sont "datées" en France, mais cela ne veut nullement dire qu'il ne s'agit pas d'une appellation officielle.

Personnellement, j'ai dû en demander un à la mairie de ma ville de naissance lorsque j'ai demandé ma carte de séjour en France en 1976...
