Good point, I see where you are coming from, Krzysztof-san. Actually, I also thought of other ways of translating 根拠 at first, as something like "basis" or "grounds" might be more suitable in some cases.

It all depends on what's there in this report - I assumed that some experiments or data collection was performed and therefore the author(s)
mentioned the results as 科学的根拠. Also, seeing that this sentence sounds like part of their conclusions or implications of this study, it appears that 対象者のアセスメント and 看護介入 have been conventionally conducted without referring to some data, and maybe providing such an empirical basis was the aim or motivation of this study.

I do think you're right in principle, but in this particular context "evidence" just doesn't sound right to me.</br>

I might of course be nitpicking, but even though "evidence" seems to be a well-established translation of 根拠 in medical texts (Mainly related to evidence-based medicine), here I would use another word. We could talk about "evidence" if we were discussing, say, efficacy of some treatment or medicine, but as far as I understand it, this sentence instead refers to assessment and intervention themselves - that's why I opted for justification/rationale.</br>

Anyway, just my two yen.

I don't see any problem with Kurt-san's original translation. As Aogara-san pointed out, the Japanese source text is rather awkwardly written, but I think Kurt-san's interpretation is on the right track; We use "evidence", "basis", "based on” etc.. to express 「根拠」; it largely depends on contexts and I'd use "evidence" in this context.
