To avoid backup withholding when selling your ESPP shares or receiving dividends, it is very important that when enrolling in the Plan you complete a W-9 or a W-8BEN Internal Revenue Service (“IRS”) form certifying your tax identification number, depending on your legal residence as explained below.

Steps for U.S. Citizen (or Resident Alien) Tax Certification
Step 1: After you logon, click on “Tax Information”. Select your status and click “Next”.

Step 2: Select whether you are subject to backup withholding, then you will be asked to certify your U.S. Taxpayer Identification Number, backup withholding and U.S. person status. Click “Next” to continue.

Step 3: Enter your PIN to affirm the information is correct and click “Submit”. Your PIN serves as a legally binding signature. For your records, a confirmation screen will appear after clicking “Submit”.
