I am translating a birth certificate and following the name of the child, it says that the declarant takes resposability for the name...?
something to this extent...

any suggestions?

I've never seen the phrase before, although I've translated many birth certificates. I think it may be simply a peculiarity of that Conservatoria.

I think the literal translation - "name for which the registrant takes/accepts responsibility" is too strong and places too much emphasis on the "responsibility" aspect. I think the original is just a fussy, legalistic way of saying "name declared by the registrant"

Explanation:
"No, the child has an ordinary name...."

Had a similar birth certificate.
In this case, the father and mother were married, but the girl received the name and surnames of the mother only. Usually the child gets the father's surname, and mother's surname as middle name.
