Obtaining permission from client to use project in portfolio - advice needed

Recently, I completed the translation of an Enrollment Form to become an independent distributor of a product.

The project included the translation and layout of the document, and I would like to include the document as part my translation portfolio.

Do I need permission from the client to do that? The client is actually a third party, since I was contracted by the marketing company in charge of designing the form.
