Source text - French La recherche recente a mis en question l'hypothese "classique" de la dopamine qui expliquait la psychose par une hyperactivite globale du systeme dopaminergique. Les conceptions modernes de la psychose soulignent l'importance d'un desequilibre du systeme dopaminergique entre le lobe frontal (voie meso-corticale) et les centres sous-corticaux (voie meso-limbique). Ce desequilibre ne suffit a tout expliquer, mais il y a un consensus que cette dysfonction du systeme dopaminergique est souvent a la base du syndrome psychotique (hallucinations, idees delirantes, pensee desorganisee, comportement "catatonique" et que tous les antipsychotiques ont un effect sur ce systeme. Pour faire le lien entre biologie et psychisme, on peut interpreter cette dysfonction du systeme dopaminergique comme une deregulation de l'attribution d'importance aux stimuli externes et internes. Ceci expliquerait la difficulte dans la communication et dans la capacite d'agir selon la propre volonte du patient psychotique. Le but principal du tratement pharmacologique est alors de retablir un equilibre entre inactivation du lobe frontal (capacites cognitives et motivation) et excitation du systeme limbique par des medicaments qui ne bloquent pas de la meme facon ces deux voies dopaminergique. C'est ce que font la clozapine et d'autres antipsychotiques apparentes, dits de seconde generation, alors que les anti-psychotiques conventionnels bloquent la voie meso-corticale et la voie meso-limbique avec la meme intensite.
Translation - English Recent research has put in question the “classic” dopamine hypothesis, which explained psychosis as global hyperactivity of the dopaminergic system. Modern conceptions of psychosis emphasize the importance of an imbalance in the dopaminergic system between the frontal lobe (mesocortical pathway) and the subcortical centers (mesolimbic pathway). This imbalance does not sufficiently explain everything, but there is a consensus that this dopaminergic system dysfunction is often at the base of psychotic syndromes (hallucinations, delusional ideas, disorganized thought, and “catatonic” behavior) and that all antipsychotics have an effect on this system. To make the link between biology and psyche, this dopaminergic system dysfunction can result in the deregulation of the attribution of importance to external and internal stimuli. This would explain the psychotic patient’s difficulty in communicating and the inability to act according to his own will. The principal goal of pharmacological treatment is thus the re-establishment of equilibrium between inactivation of the frontal lobe (cognitive abilities and motivation) and excitation of the limbic system by medicines which do not block these two dopaminergic pathways in the same way. This is what clozapine and other related antipsychotics (so-called second generation drugs) do while conventional antipsychotics block the mesocortical and mesolimbic pathways with the same intensity.
